for your first visit

The initial evaluation will be 45-60 minutes and will include an interview, physical examination, and treatment as time allows. If you you are unable to fill out your New Patient Packet ahead of time, then please arrive 10 minutes prior to your scheduled appointment to do this. Wear comfortable clothing that will allow easy access to the body part being evaluated. If you are being evaluated for orthotics, please bring a few shoes that you regularly wear and any past orthotic devices you have had. You will need to wear shorts or pants that can be rolled above the knee.

Insurance

We accept most major insurances and accept VISA & MasterCard. We will gladly bill your insurance company for you, but please be aware that patients are responsible for any deductibles, co-pays, and non-covered, or non-authorized services. In the state of Washington, clients are allowed to see a physical therapist without a doctor’s prescription, however some insurance companies require one. We therefore recommend that you contact your insurance provider prior to your first visit to determine if any pre-authorization or referral is required.

  • A valid photo ID.
  • Your insurance card, claim number, and/or billing information.
  • The prescription for physical therapy from your doctor.
  • A completed New Patient Form (for first visit only).

treatment of minors

Clients under 18 years old must be accompanied by a parent or guardian on their first visit so that authorization forms can be signed before treatment begins. After the first visit, parents can continue to attend the treatment sessions at their discretion.
